INLIV IS COMMITTED TO THE PRIVACY, CONFIDENTIALITY AND SECURITY OF ALL INFORMATION.

Personal and personal health information you provide to INLIV is collected, used and disclosed in accordance with the provisions of the Personal Information Protection Act (PIPA), and Health Information Act (HIA). INLIV also adheres to the provisions of the College of Physicians and Surgeons of Alberta. In addition, our company privacy and security policies are comprehensive. We have a designated Privacy Officer on staff and are focused on privacy with regard to the collection, use and disclosure of your information as well as the accuracy, availability and notification/communication with you around collected personal and personal health information.

INLIV maintains its records in a secure environment. The protection of information against unauthorized access, use and disclosure is maintained at the highest level. INLIV has administrative, technical and physical safeguards in place.

INLIV does not sell your personal information for any purpose whatsoever.

INLIV COLLECTION OF PERSONAL INFORMATION

Your information is collected to ensure that comprehensive medical treatment, health services, and education can be provided. As a client of INLIV, we may collect demographic, health, medical, contact, insurance and payment information. All information is collected with your consent

If you are an employee of an employer who retains INLIV to provide you with health services, we may collect personal information as well as information about your job. INLIV may share aggregated statistics with your employer; however, your personal information cannot be identified from these aggregate statistics. We are committed to the privacy of your information and have safeguards in place to ensure anonymity and privacy.

INLIV USE OF PERSONAL INFORMATION

Information collected by INLIV is used to assist in the delivery of our client care, programs and services. In addition, this information is also used to communicate with you regarding other INLIV products, services, events, medical updates and educational information. INLIV may use the information collected to create anonymous statistics to aid service and business analysis. These statistics are created blind and do not contain personally identifying information.

INLIV DISCLOSURE OF PERSONAL INFORMATION

Within INLIV, access to information is strictly controlled. Personal and personal health information is disclosed on a need to know basis to staff that require the information to perform their assigned duties to ensure high quality care. Only the least amount of health information necessary to carry out the intended purpose is collected, used and disclosed.

With your written consent, we will forward information to your family physician or another third party service provider to ensure appropriate ongoing monitoring and long-term follow up. Your information will not be forwarded without your consent.
